Our Community-based Residences—sometimes referred to as group homes—offer adults with intellectual and developmental disabilities the opportunity to live as independently as possible in the communities of their choice. Nestled in residential neighborhoods, our Community-based Residences offer 24/7 staff support, expert clinical services, and opportunities for individual growth.

Individualized Service Planning
When someone is ready to move into one of our homes, we take the time to understand their needs and interests. Our homes are staffed with Direct Support Professionals (DSPs), nurses and other human services professionals who develop Individualized Service Plans (ISPs) to help each individual reach new milestones.
We offer personalized services, including:
• Individualized service plans
• 24/7 on-call support
• Medication management
• Community integration
• Management of daily activities
• Structured activities
• Life skills development
We take pride in the quality of our programs. As with all of our services, our community-based homes are licensed by Florida’s Agency for Persons with Disabilities (APD) and are carefully monitored by our quality improvement team to ensure the health, safety and happiness of each person we support.
